Jazz On The Lawn

PERMISSION & MEDIA RELEASE

Congratulations! You have been invited to attend Callanwolde Fine Arts Center’s Jazz on the Lawn event. This is an opportunity for students to meet and connect with featured, professional artists, and experience live jazz while celebrating an evening of music and arts. As part of the experience, students will have the opportunity to interact with members of the Support Young Artists All-Star Media Team, operated by The Focus Group Movement, LLC, who will document the event through photography and videography. Please review the information below and give your consent for your child to participate in the event and be included in photo and video coverage.

PARENT/GUARDIAN CONSENT

By completing this form, I understand that:

• My child will participate in the Jazz on the Lawn experience as part of an educational media opportunity.

• Activities may include photography, videography, editing, storytelling, content creation, and creative production.

• My child will remain under the supervision of approved adults designated by The Focus Group Movement and/or the host organization.

• My child's transportation to and from the event is solely my responsibility.

• My child is expected to conduct themselves professionally and respectfully.

• Participation is voluntary, unpaid, and may be discontinued by either party at any time.

MEDIA/INTELLECTUAL PROPERTY

I grant permission for:

• Callanwolde Fine Arts Center

• The Focus Group Movement, LLC

• Support Young Artists

to use submitted photographs, videos, recordings, and creative works for educational, promotional, fundraising, marketing, print, digital, website, social media, and archival purposes without additional compensation. I understand that this permission is perpetual, royalty-free, and non-exclusive. My student may also use created work in their professional portfolio, college applications, scholarships, and resumes.

LIABILITY ACKNOWLEDGEMENT

To the fullest extent permitted by law, I release and hold harmless:

• Callanwolde Fine Arts Center

• The Focus Group Movement, LLC

• Support Young Artists

• Their employees, volunteers, and representatives

from claims arising from ordinary participation in this volunteer program, except where prohibited by law or caused by gross negligence or willful misconduct.
